All proof documents will need to be uploaded in a digital format (pdf, jpg, jpeg).

Please ensure you have the following information and documents ready before you apply:

A valid e-mail address

You will be asked to enter a valid e-mail address. We will e-mail you a link to set-up a password for an account. You will be able to track the progress of your application or make amendments to your application through your account. If you don't have a valid e-mail address our Concessionary Team are available to help. Please contact us on 0118 9746000 and ask for Concessionary Travel for advice/assistance.

A Recent colour photograph of yourself (passport size)

  • Your photograph must be in the following digital file format (jpg, jpeg).
  • During the registration process, you will be able to take a photograph using a webcam or upload one from your computer.

Proof of Identification - please provide one of the following:

  • Birth / adoption certificate (along with deed poll / marriage certificate if name has changed)
  • Current passport - the identification page. If your passport is not an EU passport, a copy of the visa page is also required
  • Current driving licence - the counterpart of a driving licence is no longer a valid proof of identity
  • Printed prescription showing your date of birth

Proof of address - please provide one of the following:

  • Council Tax / utility bill
  • Bank statement
  • Driving licence

Proof of disability

If you fall under the automatic eligibility category we will need to see a copy of a current award letter.
For the discretionary eligibility categories, read the relevant guidance under 'eligibility' (above) for details of what proof we require. The form can also be used to obtain a confirmation from an independent medical professional.
